The Advanced PCN Pharmacist will work as part of the PCN multidisciplinary team to deliver and develop all elements of programmes which involve the optimisation of medicines for the patients of Denton.

Leading a team of PCN Pharmacists, they will work collaboratively with the PCN Clinical Director and Manager to ensure that pharmacy services are delivered to the highest possible standards.

Across Tameside and Glossop we have the benefit that our PCN teams do not work in isolation and support is offered from other Advanced Primary Care Network Pharmacists across the conurbation as well as the Hospital Pharmacy service.

Main duties of the job

The Advanced PCN Pharmacist will work as part of the multidisciplinary team to deliver and develop all elements of programmes which involve the optimisation of medicines for their allocated PCN.

Responsible for working with their assigned GP surgeries delivering and developing a clinical pharmacy medication review service to community and care home based patients, in particular those identified as being at high risk of hospital admission or otherwise vulnerable.

Collaborate with PCN Clinical Director and Manager to ensure PCN Pharmacy services meet the clinical aspiration of the assigned PCN.
Manage and develop PCN Pharmacist(s) within their team.

Detailed job description and main responsibilities

Working Across Interfaces
Facilitate communications between GPs, hospital doctors and pharmacists,
community pharmacists and community nurses to ensure all key stakeholders are aware of specific patient medication related issues and the actions required to optimise individual patients’ pharmaceutical needs.
To develop referral processes and pathways in conjunction with health and
social care professionals
Facilitate discharge by managing in-reach to liaise with Trust ward based
pharmacist teams, to reconcile medicines following hospital discharge or outpatient consultation
Ensure patient safety when patients are transferred between care providers through reconciliation of prescribed medicines
Arrange and manage systems to ensure continuity of medicines supply to high risk groups of patents.
Liaise with colleagues in community pharmacy to align support for medicines adherence such as Medicines Use Review (MUR) and New Medicines Service
(NMS)
Review patients at higher risk of unplanned hospital admission, analyse
complex patient information and apply highly specialist clinical knowledge with the aim of making adjustment to medicines to reduce this risk and to ensure optimum clinical outcome, using judgement where information is not readily available
Reduce inappropriate poly–pharmacy and waste through clinical medication review and medicines optimisation and promote cost-effective evidence based prescribing
To provide clinical and managerial leadership to GP practice teams in respect of medicines optimisation issues by attendance and presentation at staff
meetings, tutorials, training sessions, external courses, reflective practice and clinical supervision.
To identify any medication related trends / themes / issues / incidents within the networks and liaise with and communicate those to the network pharmacists and other healthcare professionals, and lead on the implementation of any changes required to minimise future incidents
Attend multi-disciplinary meetings relevant to patients referred to the service

To assess and facilitate patient adherence and self-administration capability with prescribed medicines and promote self-care as applicable.
Uses inter-personal skills to counsel patients to ensure they gain maximum
benefit from their medicines, and educating them regarding the use of
equipment for the administration of medicines
Development of patient-centred pharmaceutical care plans
Manage the audit agenda as directed by the PCN, feedback the results and be responsible for implementing changes to support medicines quality
improvement
Work as an Independent Prescriber, prescribing from within your sphere of
competence (if not already qualified as an Non-Medical Prescriber (NMP) the post holder is expected to undertake a Non-Medical Prescribing training course) Medicines Information; Training and Development
Identify and support the training needs of co-workers and carers, and develop and deliver programmes to meet those needs. Organise and provide medicines related training to other members of the PCN Pharmacy team, GPs, and their staff.
To attend and fully participate in quarterly performance reviews with designated clinical supervisor.
Participate in all mandatory and recommended training programs (including CPPE Primary Care Pharmacist Education Pathway (PCPEP)
Answer medicine related enquiries from GPs, other practice staff, PCN
colleagues and patients, suggesting and recommending solutions and provide follow up for patients to monitor the effect of any changes. Ensure responses are tailored to the patient’s medical ,social and cultural needs
Keeps abreast of current pharmaceutical practice and knowledge, attending meetings, courses or conferences as necessary, for continuing professional development / revalidation in accordance with national guidelines from the Royal Pharmaceutical Society and / or General Pharmaceutical Council

Communications and Leadership
To lead and develop the PCN Pharmacy service provided to assigned PCN
Working with GP information technology systems, especially patient medication record (PMR) systems including EMIS Web and Doc Man and pathology results systems such that you are able to ascertain relevant patient clinical data to support comprehensive medication reviews. Being proactive in ensuring all relevant patient information is obtained prior to patient review.
Demonstrate an ability to influence GPs / practice staff / other PCN Team
members and patients. To ensure that self and others are motivated to achievethe required goals.
Demonstrate an understanding of national priorities and their implications and impact on medicines optimisation within the networks
To demonstrate understanding of the pharmacy role in governance and be able to implement this appropriately within the PCN
To frequently communicate highly complex drug or medicine related
information to patients and carers tailoring the information to the patient’s
specific needs.
To communicate highly complex drug, financial or medicine related information effectively to all grades of NHS staff, in line with the needs of the department, clinical area or Acute Trust.
To provide high quality patient education on medicines use. This includes
formal presentations to large patient groups as well as individualised
information.
Able to analyse and accurately interpret data from GP prescribing systems and prescribing data from a range of sources including ePACT, Greater Manchester Business Intelligence Tool and Trust I.T. systems. Able to use such information sets to provide advice to optimise medicines usage
Record all relevant pharmaceutical information in the patient’s medical notes (paper or electronic) and interventions in line with best practice. This includes communication in writing with other agencies e.g. GPs, community
pharmacists, CCG Medicines Management Team and NHS England where
required
Liaise directly with practice managers / GPs / nursing and care home staff with regard to any required medical patient information / access to GP information systems etc.
Identify and address sub-optimal practices including repeat prescribing
performance in order for it to become in line with best practice.

Clinical Standards
To support practice prescribing such that it is in line with GMMMG, NICE and MHRA guidance and generally to support improvement in the quality, safety and cost effectiveness of prescribing across the health economy
To ensure adherence to the GMMMG Joint Formulary and local formularies
including Trust Medicines Formulary
To implement changes to medicines that arise from MHRA alerts, product
withdrawal and other local and national guidance
To monitor Tameside & Glossop community based non-medical prescribing
against the GMMMG formulary

Analysis and Audit
Record and capture activity information for agreed data sets and present this in such a way as to allow effective appraisal of the scheme. In particular to provide regular feedback to commissioners and other key stakeholders regarding agreed key performance indicators (KPIs), clinical interventions, activities and service / patient outcomes
Undertakes and supervises practice research and clinical audit.

Service Development
Be flexible in approach to develop and improve the PCN Pharmacy service,
responding positively and in a timely way to those changes which may occur /be suggested and affect the nature, direction and delivery of pharmacy services in this area. Contribute pharmaceutical advice associated with the development and implementation of PCN Pharmacy service developments
To participate in discussions concerning the operation and development of the IN service.
To identify and implement service developments and consult with the relevant senior clinical personnel to ascertain the impact on network services and required action

  • Vocational Masters degree in Pharmacy ( 4 years) or recognised equivalent
  • Registration with General Pharmaceutical Council
  • Completed or undertaking: a post-graduate diploma / MSc in clinical pharmacy or CPPE Primary Care Pharmacist Education Pathway
  • Independent Non-medical Prescribing Qualification ( or commitment towards training for same)
  • Previous experience of working with patients with long term conditions / complex needs
Desirable criteria
  • Membership of the Royal Pharmaceutical Society
